Appendiceal Inversion:

The image is of a inverted appendix. This finding is uncommon and if it is not on the differential can lead to mismanagement as endoscopic removal is associated with high rates of peritonitis. The finding is associated most often with history of open appendectomy but can also occur spontaneously. Appendiceal inversion can be differentiated from other more concerning entities by appearance, history including CT images and surgical history.
